We are open from 8:30 am  until 5 pm, Monday through Friday.  While we are in the office on Thursday afternoons we try to reserve this time for completion of administrative tasks and we do not schedule appointments on Thursday afternoon.

Handicapped Access

There are 2 elevated parking spaces next to the building designated for patients with physical handicaps.   In addition, there are 3 designated handicapped parking spaces just north of the main driveway.   A wheel chair ramp is available on the south side of the building.  This can be accessed from the south driveway.

After Hours Contact Information

Our office phone number 850-224-7154 is answered 24 hours daily seven days per week.  After our regular business hours messages may be left.  If you have a life threatening medical emergency, call 911.

For medical questions and problems that will not wait until the next business day, speak to an attendant at our answering service who will forward your call to the endocrinologist on call.

Prescription Refills

Please allow us 2 work days to refill prescriptions.  When refills are needed, please contact your pharmacy which will in turn contact us with your request.

Please call us

  • If your diabetes control has deteriorated suddenly such that frequent high or low blood sugar is a problem.
  • If you have been in the hospital: Hospital stays frequently necessitate a change in diabetes regimens.

What to bring on your first visit

  • A government issued picture ID, insurance card, and copayment.

If you have diabetes

  • Bring you glucose meter or blood sugar  log and your pump if your wear one.
  • Most recent labs you may have had done by another physician

If you have thyroid problems

  • Bring thyroid ultrasound images on film or CD (not just reports)
  • Thyroid Scan(s)
  • TSH, free T4
  • Any operative reports and pathology reports of thyroid biopsies or surgery

If you have a pituitary disorder

  • Bring MRI scan images (CD or film) - not just the written report.
  • Any recent blood tests of pituitary function: TSH, free T4, LH, FSH, cortisol, ACTH, prolactin, testosterone (men only), IGF-1
  • Operative reports, path reports (if you have had pituitary surgery)

If you have osteoporosis

  • Bring your DEXA Scan (complete report – not just the summary)
  • Any blood test results your primary doctor has ordered
If you are unable to keep your appointment

Please notify our office 48 hours prior to the appointment date to reschedule. We reserve the right to bill a $50.00  fee for  appointments missed without notification.
